1. Official Team Roster (EVENT ROSTERS NOT ACCEPTED)
2. Player ID Cards
3. Birth Certificates (ALL guest players)
4. Guest Player Forms
5. Medical Release Team Waiver (Do not upload individual medical releases)
6. Permission to Travel (Teams traveling from outside of Oklahoma - Not required for US Club)
